New Patients
How to get the most out of your first visit
When you make an appointment, we will e-mail new patient forms to be completed before your appointment time.
Check-in times are 15 minutes prior to your appointments. This allows time for any necessary paperwork to be completed. Come to each visit with valid ID and insurance card. Co-pays will be collected at the time of check-in for your appointment. If you arrive late, we will give you the option to reschedule or to wait and be seen the same day, when Dr. Pollard is available.
It is also important that Dr. Pollard have imaging discs to review, if you have had any x-rays, CTs, or MRIs of the painful area. The report is nice to have, but the disc is much more important.
Established Patients
When you return for follow-up
Return appointments are scheduled for a shorter time slot than first visits.
Return visit forms will be e-mailed to you. Please complete before your appointment.
Check-in times will be scheduled 15 minutes before your scheduled visit with Dr. Pollard to allow time for any forms necessary. If you are 15 minutes, you will have the option to reschedule or wait until Dr. Pollard has a time to see you that day.
It is important to think of your response to prior pain treatments before arriving. You may even find it helpful to make notes for quick reference.
